- Records the causes considered as well as the root cause found
- Links each root cause to the action taken
Who Needs a Root Cause Analysis Register?
Organisations working to ISO 9001, ISO 14001, ISO 45001 or another management system standard that want a simple record of root cause analysis for significant issues, repeat problems or trends found at review.
Using the Register
Complete a row when an issue is significant or keeps recurring. Work through the possible causes before settling on the root cause, record the action taken, and check at a later review that the action worked.
Physical, Human and Organisational Causes
The guidance row in the register prompts you to list every possible causal factor before deciding on the root cause, under three headings:
- Physical causes - problems with materials, equipment or climatic conditions
- Human causes - mistakes, lack of skills or poor decision making
- Organisational causes - procedural failures or a lack of procedure
It also prompts you to review whether the remedial action has addressed the root cause, and to raise a new issue if further action is needed.
Document Control
The footer carries the register number and title, issue, information classification and page numbering. The register can be listed with your other controlled documents on the F-IMS20 Document Register.
Works with the Issues and Actions Register
